Erectile difficulties – often the first sign of something else

Gradual onset is usually a blood vessel problem, which makes it an early warning for heart disease and diabetes rather than only a sexual issue.

Why this is a general health article

An erection depends on blood flow through small arteries. Those arteries narrow before the larger ones supplying the heart do, which means erectile difficulty that comes on gradually is frequently the earliest visible sign of vascular disease — often years before a heart attack.

That reframing is the most useful thing on this page. It is a reason to go and be checked for blood pressure, cholesterol, blood sugar and smoking risk, not a reason to quietly order tablets online.

The pattern tells you a lot

  • Gradual onset over months or years, with no morning erections, points towards a physical cause — blood vessels, diabetes, hormones or medication.
  • Sudden onset, with normal morning erections and situation-dependence, points more towards stress, anxiety, relationship factors or depression.
  • Most cases have elements of both, because the two feed each other.

Common contributors: diabetes, high blood pressure, high cholesterol, smoking, heavy drinking, being significantly overweight, low testosterone, prostate surgery, and a long list of medicines including some blood pressure tablets, antidepressants and the prostate-shrinking drugs.

Treatment

The tablets work well for most men, and they work by improving blood flow — they do not create desire, and they need arousal to do anything. They are prescription medicines in South Africa for good reason.

Prescribed after assessment:

  • Sildenafil Viagra, Revatio, Actigra

    Erectile dysfunction; pulmonary arterial hypertension (Revatio)

  • Tadalafil Cialis, Adcirca

    Erectile dysfunction; benign prostatic hyperplasia; pulmonary arterial hypertension

Other options exist where tablets do not suit or do not work: vacuum devices, injections and implants among them. A doctor can walk through these.

What helps beyond a tablet

  1. Stop smoking. It has a direct and substantial effect here, which motivates some men more than lung disease does.
  2. Get blood pressure, blood sugar and cholesterol checked, and treated if needed.
  3. Cut back on alcohol.
  4. Lose weight and get active — physical activity improves erectile function measurably.
  5. Address sleep, particularly if you snore heavily; untreated sleep apnoea contributes.
  6. Treat depression and anxiety if present, and consider talking therapy where the pattern suggests a psychological component.

Common questions

Is it just part of getting older?
It becomes more common with age, but it is not an inevitable part of ageing and it is not something to accept without assessment — precisely because it often signals treatable vascular disease or diabetes.
Are the tablets sold online or in shops the same thing?
Frequently not. Products sold outside pharmacies have been found to contain the wrong dose, no active ingredient, or undeclared drugs. Combined with nitrates they can kill. Get a prescription — the consultation is also what finds the underlying cause.
Should I get my testosterone checked?
It is worth testing if there is also low libido, fatigue, loss of morning erections, reduced body hair or shrinking testicles. Low testosterone is a less common cause than vascular disease, and testosterone treatment is not a general remedy for erectile difficulty.
